- IV phenytoin preparation contains propylene glycol, which promotes hypotension when injected rapidly.
Clinical Pearls

Induces CYP enzymes; associated with resistance to nondepolarizing NMBAs and increased opioid requirements. Fosphenytoin lacks propylene glycol and can be given 3x faster.
